Such COI must contain the criteria provided below: COI must be for person or entity that will be on the premises performing the removal or disassembly. Note: Most items still connected and need disconnected with NO STRUCTURAL DAMAGE during removal. No torches allowed. Any floor / ground holes MUST be safely filled or covered with gravel /plywood following removal of item. Floor bolts cut off at ground level. Wires properly terminated.
